Drug Free Workplace Premium Credit Program Online Certification

DIRECTIONS FOR APPLYING ONLINE FOR CERTIFICATION OR ANNUAL RECERTIFICATION

After reading and understanding the Rules & Guidelines for Certification of the Drug-Free Workplace Program, answer the following questions by completing the checklist. Keep documentation of compliance for your records for review by your insurer or the State Board of Workers’ Compensation upon request. Certificates are valid for one year from date of certification.

There is a $35.00 application fee payable by credit card. Upon approval by the State Board of Workers’ Compensation of application submitted, certificates can be printed from your printer. Certificate reprints are available online at no extra charge.

If you choose to apply by mail, beginning January 1, 2008, the State Board of Workers' Compensation will ONLY accept the August 2007 revised paper application for certification.
